Prayer Partner Questionnaire

During our mission week this summer, someone back home (your prayer partner) will be praying for you and keeping you in their thoughts. Each evening of our trip, you will get to open a gift bag (prayer partner bag) put together just for you by your prayer partner. This is our Prayer Partner program. Your daily prayer partner bag can include things like words of encouragement, reminders from home, prayers, well wishes, toys, snacks and surprises. So we can match you up with a prayer partner and they can know how to fill your prayer partner bags, we want to know a little more about you. Note: If we have time, we will also try to put together "volunteer cards" with the information you provide.
